Gift Certificate

$1000 Gift Certificate

We understand that it takes an extra effort to change dental lab. Sometimes dentists stay in bad relationships just to avoid the hassle that comes with this change. So, in order to help to ease the discomfort and to provide an additional benefit we are offering a $1000 Gift Certificate to a dental office to redeem while we are getting acquainted.

Rules of Redemption

Limited to new customers only
Not to be combined with any other offer
One certificate per dental office
Expiration Date of the Certificate does not apply once first case is received
Life of the Gift Certificate is 12 months from the date of the first case received
Gift Certificate to be applied to Labor charges only
Gift Certificate to be applied against the balance of the monthly statement
Gift Certificate can only be applied against 20% (maximum one time value) of Labor Charges of each monthly statement. Example: Statement balance is $2000 dollars. Labor charges are $1200 dollars. Remaining amount could be a combination of the following: shipping cost, alloy cost, implant parts cost and taxes. Maximum TEMPORARY credit which could be applied toward this statement is $240 dollars. That will reduce statement balance to $1760 dollars. This balance must be paid by the 25th of the following month.
Temporary credit will become a Permanent credit once payment is received and successfully deposited into our bank account.
To be eligible to receive remaining balance of the Gift Certificate, dentist must make all payments by the 25th of the month following the month of the statement.
We will continue applying remaining left over credit from a Gift Certificate, assuming all conditions are met, until whole $1000 is applied.
If payment is not received by the 25th of the month following the month of the statement, Dentist is not eligible to receive the remaining credit of the Gift Certificate. Furthermore, Temporary Credit which was issued earlier is removed and Dentist is responsible for the whole amount.
